A chronic disease management plan, where, using the VitalsOne device, any changes in your vital parameters will be immediately identified, leading to intervention or a suggestion for scheduling a medical appointment.
The approach is individualized for each specific chronic disease: that is, the parameters to be evaluated with the VitalsOne device, as well as the evaluation frequency, will vary according to the disease in question.
In this plan, your vital parameters are monitored remotely by a healthcare professional:
- Weekly by a Nurse or Doctor.
- Monthly by a Doctor.
You will also have access to a free specialty medical consultation every six months.
